🔎 This case follows a 61-year-old man with variceal bleeding, imaging suggestive of cirrhosis, and portal vein thrombosis. However, unexpectedly low liver stiffness and hepatic venous pressure measurements raised important diagnostic questions.
📊 Further investigation, including liver biopsy, revealed porto-sinusoidal vascular disorder (PSVD) rather than cirrhosis, illustrating how careful interpretation of elastography and hemodynamic data can fundamentally change the diagnosis.
💭 A valuable reminder that, even in patients with multiple potential liver disease risk factors, the cause of portal hypertension should never be assumed.

Hereditary haemorrhagic telangiectasia (HHT) is a rare vascular disorder that can lead to complex hepatic vascular malformations with significant clinical consequences.
🔶 In this case, a 64-year-old man with genetically confirmed HHT developed symptomatic intrahepatic shunts, resulting in portal hypertension, recurrent gastrointestinal bleeding, and high-output cardiac failure.
💡 Treatment with bevacizumab provided temporary clinical improvement, but disease progression was ultimately marked by refractory ascites, severe malnutrition, sarcopenia, and increasing frailty.
📖 This case highlights the diverse manifestations of hepatic involvement in HHT, the potential role of antiangiogenic therapy, and the importance of considering nutritional status and frailty when making complex management decisions.

💠Pregnancy in women with portal hypertension presents significant maternal and fetal challenges, requiring careful multidisciplinary management and individualized risk assessment. In this remarkable case, a woman with porto-sinusoidal vascular disorder and portal vein thrombosis developed severe variceal bleeding during pregnancy, refractory to endoscopic treatment. A TIPS procedure was successfully performed, allowing the pregnancy to continue safely and resulting in the delivery of a healthy infant at 38 weeks.
