𝗔 𝗣𝗲𝗰𝘂𝗹𝗶𝗮𝗿 𝗖𝗮𝘀𝗲 𝗼𝗳 𝗡𝗲𝘂𝗿𝗼𝗯𝗼𝗿𝗿𝗲𝗹𝗶𝗼𝘀𝗶𝘀

We present the case of a man with alcohol use disorder, initially diagnosed with metabolic encephalopathy. However, after further investigation, it was revealed to be early neuroborreliosis, a zoonosis with varied manifestations, especially in the nervous system.
🔎Neuroborreliosis can present symptoms similar to several other neurological conditions, making diagnosis a real challenge. This case highlights the importance of a comprehensive differential diagnosis, especially across the neurological spectrum of this disease.

Apresentamos o caso de um homem com perturbação do uso de álcool, inicialmente diagnosticado com encefalopatia metabólica. Contudo, após investigação mais aprofundada, revelou tratar-se de neuroborreliose precoce, uma zoonose com manifestações variadas, especialmente no sistema nervoso.
🔎A neuroborreliose pode apresentar sintomas semelhantes a várias outras condições neurológicas, tornando o diagnóstico um verdadeiro desafio. Este caso sublinha a importância de um diagnóstico diferencial abrangente, especialmente no espectro neurológico desta doença.

𝗨𝗻𝗰𝗼𝗺𝗺𝗼𝗻 𝗗𝗲𝗯𝘂𝘁: 𝗢𝗿𝗯𝗶𝘁𝗮𝗹 𝗣𝘀𝗲𝘂𝗱𝗼𝘁𝘂𝗺𝗼𝗿 𝗮𝘀 𝘁𝗵𝗲 𝗜𝗻𝗶𝘁𝗶𝗮𝗹 𝗣𝗿𝗲𝘀𝗲𝗻𝘁𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝗜𝗴𝗚𝟰-𝗥𝗲𝗹𝗮𝘁𝗲𝗱 𝗗𝗶𝘀𝗲𝗮𝘀𝗲
A 73-year-old man with 4 years of periorbital swelling, weight loss, and anorexia was diagnosed with IgG4-related disease. Imaging revealed thickening of the retroorbital muscles and soft tissue prominence, and a biopsy of the lacrimal gland confirmed the diagnosis.
🔎This case highlights the relevance of recognizing IgG4-related disease in patients with persistent and atypical orbital symptoms, helping to guide appropriate diagnosis and treatment.

Um homem de 73 anos com edema periorbital há 4 anos, perda de peso e anorexia foi diagnosticado com doença relacionada com IgG4. A imagiologia revelou espessamento dos músculos retroorbitais e proeminência de tecidos moles, e a biópsia da glândula lacrimal confirmou o diagnóstico.

𝗘𝗿𝘆𝘁𝗵𝗲𝗺𝗮 𝗠𝘂𝗹𝘁𝗶𝗳𝗼𝗿𝗺𝗲: 𝗔 𝗥𝗮𝗿𝗲 𝗣𝗿𝗲𝘀𝗲𝗻𝘁𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝗖𝗢𝗩𝗜𝗗-𝟭𝟵
A 66-year-old man presented with painful, scaly lesions on several parts of his body, including his mouth, chest, hands and feet. Skin biopsy revealed a strong inflammatory infiltrate and necrotic keratinocytes. After diagnosis of erythema multiforme (EM) secondary to COVID-19, treatment with prednisolone resulted in resolution after 3 weeks.
🩺EM is a hypersensitivity reaction, often associated with viral infections such as Herpes simplex, but can also be triggered by COVID-19, as in this case. It is important to consider SARS-CoV-2 infection in the differential diagnosis of exanthematous skin lesions, even in the absence of respiratory symptoms.

A biópsia cutânea revelou um forte infiltrado inflamatório e queratinócitos necróticos. Após diagnóstico de eritema multiforme (EM) secundário à COVID-19, o tratamento com prednisolona levou à resolução total em 3 semanas.
🩺O EM é uma reação de hipersensibilidade, frequentemente associada a infeções virais como o Herpes simplex, mas também pode ser desencadeada pela COVID-19, como neste caso. É importante considerar a infeção por SARS-CoV-2 no diagnóstico diferencial de lesões cutâneas exantemáticas, mesmo na ausência de sintomas respiratórios.

𝗣𝗼𝘁𝘁 𝗗𝗶𝘀𝗲𝗮𝘀𝗲
We present the case of a 61-year-old man, with no relevant medical history, who developed back pain for 7 months. During the investigation, a computed tomography revealed collapse of the D7-D8 interbody space, with a massive paravertebral and intracanal component, suggesting a pathological fracture. Magnetic resonance imaging confirmed the diagnosis and, after new neurological deficits appeared, he underwent decompressive surgery.
🧬Diagnosis: After isolation of Mycobacterium tuberculosis complex, he was diagnosed with Pott's disease (tuberculosis of the spine). Antibacillary treatment was started and directed to the rehabilitation unit.

𝗠𝗮𝗹 𝗱𝗲 𝗣𝗼𝘁𝘁
Apresentamos o caso de um homem de 61 anos, sem antecedentes médicos relevantes, que desenvolveu dorsolombalgia com 7 meses de evolução. Durante a investigação, uma tomografia computorizada revelou colapso do espaço intersomático D7-D8, com componente volumoso paravertebral e intracanalar, sugerindo uma fratura patológica. A ressonância magnética confirmou o diagnóstico e, após surgirem novos défices neurológicos, foi submetido a cirurgia descompressiva.
🧬Diagnóstico: Após isolamento de Mycobacterium tuberculosis complex, foi diagnosticado com mal de Pott (tuberculose da coluna vertebral). O tratamento antibacilar foi iniciado e orientado para unidade de reabilitação.
