Is a travel declaration form still required in the Philippines? Yes. All travelers to and from the Philippines are required to register through the E-Travel portal or egovph app. This is a mandatory entry requirement for all international visitors and Filipinos.
One QR code per passenger shall be used in the e-Travel/egovph System.
You may register within 72 hours (3 days) prior to your arrival into or departure to & from the Philippines.

I downloaded the egovph app, registered my husband and son in my account, and received their QR codes within minutes. Hnd na po need ng multiple accounts.
