Remember the Physical Clearance must be signed, dated, AND turned in with a COPY of your current insurance card EACH SCHOOL YEAR.

JULY 16th @ NOON is the Fall Sports Clearance Deadline.
Only 1 more month until the deadline, so schedule your doctor's appointment asap and turn in both required forms together in the PVHS Office this week! Make sure your online registration is for the 2025-2026 school year!
