We’ve been saying this

For thousands of years, women commonly gave birth in upright positions such as squatting, kneeling, sitting or standing. Squatting can increase the diameter of the pelvis by at least 2.5 centimeters, or about one inch, giving the baby more space to move through the birth canal.

Upright positions also allow gravity to assist the baby's downward movement and may support more effective uterine contractions. In contrast, lying flat on the back can allow the weight of the uterus to compress major blood vessels, which may reduce blood flow and oxygen delivery.

A review of 25 studies involving more than 5,200 women found that women who remained upright and mobile during labor, rather than confined to lying in bed, had a lower risk of Caesarean birth, used epidural pain relief less often, and were less likely to have their babies admitted to neonatal care.

Other research cited in the BBC report links upright labor and birth with shorter labor, less maternal pain, fewer forceps or vacuum-assisted deliveries, and fewer episiotomies. These findings apply to upright and mobile positions generally—not specifically to squatting—and the safest position can depend on the individual pregnancy and clinical circumstances.

The United States has the worst maternal mortality rate in the developed world, and Black women die at several times the rate of white women. This is not a rumour. It is the statistic that hangs over every Black woman having a baby in America.

Midwife-led care measurably improves outcomes. It is also expensive and rarely covered.

Kelly Rowland is one of the founding funders of birthFUND, which simply pays for midwives for families who cannot afford them. It has raised more than $3 million.

That is the whole model. No awareness campaign, no ribbon. Somebody's midwife, paid for.

She has two sons. She has been open that this is not an abstraction to her.
