https://forms.gle/eg9NKikeZXLAqwn4A

Our goal is to gather input from Park Elementary School parents and guardians. As many of you know, we recently formed a behavior team in partnership with Arkansas Behavior Support Specialists and the state of Arkansas. Together, we are developing a Behavior Support Plan to promote a safe, respectful, and successful learning environment for all students.

All responses to this form are confidential. They will be viewable by the behavior team and shared with school staff in summary form only for the purpose of informing and supporting our plan. Feedback will not be connected to individual names or students when communicated to staff, unless you choose to provide identifying information.

We sincerely appreciate you taking the time to share your thoughts. Thank you from our Park Elementary Behavior Team for your partnership, support, and commitment to helping our students grow and succeed.
