Federal investigators say stolen identities purchased online were allegedly used to create fake hospice patients in one of the most disturbing Medicare fraud schemes uncovered in years.
According to investigators, 14 hospice companies operating through straw owners enrolled identities belonging to people who didn't even live in California.
Real names.
Real Medicare numbers.
Fake patients.
Authorities say the scheme generated $267 million in fraudulent billing.

Raids across Southern California led to the seizure of $757,000 in cash and two handguns.

Many victims reportedly had no idea their identities had been used until investigators contacted them.

People were allegedly paid $300 to pretend they were dying.

Federal authorities arrested eight people accused of running a sham hospice network that allegedly defrauded Medicare of more than $50 million.
Prosecutors say healthy individuals were recruited, kickbacks were paid, and stolen identities were used so fake hospice companies could bill Medicare thousands of dollars each month for end-of-life care that patients never needed.
Authorities say the money was then spent on luxury cars, homes, and international vacations.
Meanwhile, investigators say real dying patients struggled to access legitimate hospice programs because the system had become clogged with fraudulent ones.

The operation was called "Operation Never Say Die."
And many people believe the name says everything.

A "family festival" in suburban Kansas City was overrun with "teens" who began BEATING POLICE OFFICERS and UNLEASHED BEAR SPRAY on attendees.

The fire department kept coming back to treat people who couldn't breathe.

Six new people have been charged with defrauding child nutrition, autism, and housing stability programs in Minnesota.

"What we see in Minnesota is not a handful of bad actors committing crimes. It's a staggering industrial scale fraud," said Joe Thompson, First Assistant U.S. Attorney.

The state has identified 14 programs as "high risk," costing taxpayers $18 billion since 2018. Thompson noted that a significant portion of this amount could be fraudulent, estimating that it might be half or more.
