The dental outreach van will be available to all preK-12 students on September 18th. Parents must complete a consent form by September 12th if they would like their child to participate. Consent forms have been sent home with all elementary and middle school students. High school students can pick up a consent form in the MS/HS office. Parents can also register using the QR code below.
The vision outreach van will also be available to preK-12 students, but they have to be seen by the dental outreach van in September to be able to participate in the vision outreach van on December 15th. Consent forms are the same for both programs.
