State health inspectors and the Tennessee Bureau of Investigation (TBI) are actively investigating a severe medication mix-up at Ascension Saint Thomas Hospital Midtown in Nashville.

The investigation was launched in August 2026 after a critical error inside the hospital's pharmacy. Syringes intended for routine joint replacement surgeries were filled with the wrong medication.

Key Details of the EventThe Error: Instead of receiving a standard numbing anesthetic called mepivacaine (or bupivacaine), four patients undergoing routine joint operations on August 14, 2026, were mistakenly injected with potassium phosphate (or potassium chloride).

The Consequences: The injection caused severe neurological and health complications. One patient, 72-year-old Glenda Dorton, was left completely paralyzed from the chest down following what was supposed to be an outpatient knee replacement surgery. Families reported that another patient was also left paralyzed, and a third was admitted to the intensive care unit (ICU).

The Hospital's Response: Hospital President and CEO Dr. Shubhada Jagasia issued a public apology to the victims and their families. The hospital stated that it self-reported the event to state regulators on the day it occurred, quickly identified the issue, and has since put corrective safety protocols in place.

The Tennessee Health Facilities Commission and the TBI are continuing their separate regulatory and criminal reviews to determine how the breakdown in pharmacy safety protocols occurred.

The DV-2026 Visa Bulletin numbers for Africa are looking good, but unfortunately, numbers alone mean nothing if interviews are not scheduled and visas are not issued.

With the visa restrictions affecting 75 countries and the global DV processing freeze, many selectees are left waiting and uncertain.

The numbers are moving, but will selectees get their visas before September 30, 2026?

On August 21 st, a U.S. judge struck down a Trump administration policy suspending the issuance of visas to applicants from 75 countries, saying it’s patently unlawful" and ran a foul of federal immigration law.

However the DV lottery is also affected by “The global Diversity Visa (DV) freeze”. This is an official U.S. government policy that has completely halted the printing and issuance of all green cards through the lottery program worldwide.

The freeze applies to every single lottery winner, no matter what country they are from. Why Was It Put in Place:

The reason given: : In December 2025, Department of Homeland Security (DHS) Secretary Kristi Noem ordered the pause following a fatal mass shooting at Brown University and MIT. Officials stated the suspect entered through the Diversity Visa program, prompting an immediate national security pause to review screening and vetting procedures.

Security review: The government claimed it needed to halt the program to inspect and "strengthen" screening and background check procedures.

How Does It Work in Practice: Interviews still happen. The State Department allows embassies to hold lottery interviews and accept documents but no Visas Are Printed. Once the interview is done, the visa is not issued. Your case is placed into an indefinite hold.

Next Lottery is Blocked: Registration for the next lottery year (DV-2027) is also paused and has not been allowed to open.
