INTERN CASE NOTES | CASE 01 🩺

You met this patient in a previous post: a 7-year-old MN German Shepherd with progressive weight loss, a ravenous appetite, large-volume stool, severe muscle wasting, and a BCS of 1/9.

So what did I do next?

We pursued a GI-focused diagnostic workup to investigate maldigestion vs. malabsorption, with the results ultimately supporting exocrine pancreatic insufficiency (EPI).

In this post, I’m walking through the key diagnostics, how they helped reach the diagnosis, and the general approach to management.

Clinical pearl: A ravenous appetite doesn’t always mean a patient is receiving adequate nutrition. When weight loss persists despite a good or increased appetite, think beyond simply “not eating enough.”

This case is inspired by a real case seen during internship. No personal/client identifying information is being shared.

September called for Cosmic Pink 💗✨ and honestly, what better way to celebrate my birthday month than looking pretty in pink?!

I celebrated my birthday on September 4th (and spent part of my birthday weekend at work 👩🏾‍⚕️😂), but at least I got to do it in my favorite scrubs! 🎀🩺 The birthday balloons made their appearance too 🎈

🩺 just a quick life checkin about internship life so far…

I’m about a month and a half in and so far I’ve rotated through ER (day and night shifts), Soft Tissue Surgery, and now Internal Medicine. It’s been challenging, fast-paced, and definitely pushed me outside of my comfort zone — but I’m learning SO much. The ER is no joke, but every shift has given me an opportunity to learn something new, do something I haven’t done before, and grow a little more confident in myself as a doctor.

I’m reminding myself that growth is uncomfortable, and tough seasons don’t last forever. I have a lot of this year left, so for now, I’m just taking it one shift, one case, and one lesson at a time. 🤎

A little photo dump from my first couple months in a new state 🫶🏾

We’ve officially adjusted to our new home, I’ve found some really cute local spots, had some very good food, explored a state park + waterfall, and spent lots of time adventuring with my mom while she was here helping me get settled.

This is my first time living out of state and away from my family, so this whole experience has definitely been an adjustment. But honestly? Alabama has been good to me so far. 🤎If you’re following along in my journey, I’m here for a small animal rotating internship at Auburn University so I’ll be here for one year. We’ll see where this little adventure takes me… but for now, I’m enjoying the new places, new memories, and figuring out this whole “adulting in a new state” thing one day at a time. 🌻
