Please review the following requirements carefully before attending registration.

✅ Who May Register a Student?
Only a parent or legal guardian may complete student registration. No other individuals will be permitted to register a student.

🏠 Proof of Residency Requirements
Parents/guardians must provide two acceptable proofs of residency in the legal guardian's name. (See flyer)

🚫 The following documents will NOT be accepted as proof of residency:

Voter registration cards

Utility bills in anyone else's name

Phone bills

Internet bills

Check stubs/pay stubs

Medical bills or statements

Vehicle registration or car tag receipts

Affidavits

⚠️ Registration will not be completed if:

Your two proofs of residency show different addresses.

The documents are not in the legal guardian's name.

The documents do not show residency within Carroll County School District boundaries.

💉 Mississippi Form 121 (Certificate of Immunization Compliance)

A current Form 121 is required for:

All students enrolling in a Mississippi school for the first time (K-12)

Students transferring from another state

All students entering grades 7-12

Students whose previous Form 121 was temporary or has expired

Please make sure all required documents are available before arriving for registration to help ensure a smooth process.
