A 41-year-old indian South African male presented to his general practitioner with a one-week history of fatigue, loss of appetite, nausea, abdominal discomfort, dark urine, and yellow discoloration of his eyes.

The patient had recently returned from a three-week holiday in India. During his travels, he frequently consumed street food and beverages from local vendors. He reported eating fresh salads, cut fruits, and food prepared under varying sanitary conditions. He had not received a Hepatitis A vaccination prior to travel.

Approximately four weeks after returning to South Africa, he began experiencing general malaise and fatigue. Over the following days, he developed nausea, mild fever, abdominal pain in the right upper quadrant, dark urine, pale stools, and jaundice.

The swelling initially involved the foot and ankle but gradually extended up to the thigh. He reported heaviness, discomfort while walking, recurrent skin infections, and increasing difficulty performing daily activities.

The patient recalled multiple episodes of fever associated with painful swelling of the affected limb during the early stages of the disease. He lived in an area known for mosquito-borne illnesses and had no significant past medical history.

A 17-year-old male presented to the clinic with a three-week history of an itchy rash affecting both sides of his groin. He reported that the itching worsened after cricket games where he is excessively sweating. The patient denied any pain, fever, urinary symptoms, or recent use of new soaps or detergents. He was otherwise healthy with no significant past medical history.

On examination, a well-demarcated, erythematous rash with a slightly raised, scaly border was noted in the bilateral inguinal regions. The sc***um was spared. There were no signs of secondary bacterial infection.
