Components of a nursing care plan.
1. Problem.
This is driven from the signs which the client may present with and they make you determine the problem.
Note: there is a difference between a sign and a program e.g When the client presents with high temperature, the problem is altered body temperature and not fever.
2. Nursing diagnosis.
This shows how you reached the problem and it has 3 things
a) the problem itself
b) What is causing the problem
c) What is helping you know that there is that problem (Sign)
E.g . Altered body temperature related to infections or inflammatory processes evidenced by fever.
3. The goal.
This is What you want to achieve after you identify the problem and it has 3 parts
a) What you want to achieve
b) The normal ranges of what you want to achieve
c) for how long are you going to achieve
E.g . To restore body temperature to Normal ranges of 36.5 to 37.5 within 3 hours of hospitalization.
4. Nursing interventions.
These are measures you are going to put in place for you to achieve your goal. Remember to start with nonpharmacological then pharmacological ones.
E.g . I will reduce the clothing of the client to promote heat loss through
convection
I will administer prescribed antibiotics Such as x-pen to clear infections.
5. Evaluation.
This determines whether the goal has been achieved or not and in theory Always a goal is achieved and it has three parts
a) What you have achieved
b) for how long have you achieved
c) how do you know you have achieved.
E.g . Body temperature restored to Normal ranges within 2 hours of hospitalization evidenced by body temperature of 36.3
